Scouts BSA participants must fill out and sign along with their parent(s).
Turn in all to Ms. Amy Taylor (Annual Form Coordinator) by October 18th, 2022. Failure to do so will result in a suspension of activity until that issue is rectified.
Please make a copy to retain before turning in the forms.
